New Patient Registration

The doctors welcome new patients who live within our practice area.

The practice has a boundary that is agreed by the PCT. If you live within the practice boundary and wish to register at the practice, please come into the surgery of your choice and speak to reception. The practice boundary is available if you would like to have a look, just ask at reception

To Register

You will be asked to complete an NHS registration form and a new patient questionnaire. All patients need to fill out the registration form and there is a separate questionnaire for children aged 5 years and under. Once you have completed your forms and handed them back into the practice, you need to make an appointment for a New Patient Health Check. This is a 20 minute appointment with the practice nurse or health care assistant (HCA). You will be asked to give details of your medical history and medication. This check is  practice policy and is important in order to keep your medical  record up to date.

You will be registering with the practice and not with a specific GP. You are free to see which ever clinician you choose.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
